You left the city and location out (not the specific address but the location - ie Near Coit & George Bush), the phone is nice to include as well as their website, even if you found it on ECCIE, you can cut & paste from someone elses review. Also, perhaps a few more details on your activities and any comments about the therapist, what was she wearing? was she friendly or standoffish. Did she give the idea that she really enjoyed being with you or did she act like she wished she could get this over with?
