The protocol for the title “Dr” is it is used when the holder of the non medical PHD degree is representing their field of expertise.

Medical Doctors can be referred to as “Doctor” on all occasions.

It just shattered my expectations of what 70 year old women typically wear. It's a free country, she can wear what she wants. I just typically expect a woman her age from the upper east coast to shop at LL Bean, not Forever 21.

I worked with a woman who had her doctorate in Education. Nobody called her Doctor. Its fairly recognized those Doctorates are the easiest to achieve. A buddy from HS and college worked his butt off and was accepted into medical school. His wife had a 3.5 gpa. He said that she was an Education major and her homework literally consisted of middle and high school level work because that was what she was going to teach.
