First off, show that it's fraud and not mismanagement.

If it's fraud, prosecute someone.

If it's mismanagement, replace someone.

Also address how much of that was recovered and the cost of recovery. If we want things to run like a business, determine the amount of acceptable shrink/loss. If you expect that to be zero then I hope to God your cost of recovery is minimal.

I didn't realize this but there are a lot of very old people in this country collecting social security. People are living up to 159 years old. Really! Who is collecting this money? DOGE is kicking you know what.

Are there mistakes in Social Security Administration? YES. Humans make mistakes (to err is human, to really muck it up takes a machine)

Are there some waste/mismanagement? YES, I'm sure there's some.

Are there "tens of millions" of 150 year olds collecting Social Security?

When my dad died, SS knew and reached into his bank account and took back the money they gave him at the first of the month.

No hospital, probably the coroner I'm guessing. SS knew, the Navy (retirement) knew. That's why I don't understand all these payments going out for dead people.
