I see that your visit were a couple of weeks ago. When I tried to visit last weekend (7/14) I was asked to fill out this long intake form with all kinds of personal information.

There was also a drawing of a body on the form where one could indicate what area needed attention. However the area where I needed extra attention was missing from the picture, so I left.

I think they might have been told to do things per the MS State Board of Massage's regulations, or else...

Has anyone else come across this?

Jb, I've never heard of that before. I'm thinking they are doing that because of all the spa busts that's going on, a way to show they are more legitimate.

I have often been asked by Mamasan if I wanted a massage

I always thought it was more than conversation, trying to confirm that I was a prospective client and getting me on a recording saying I wanted a massage.

NEVER have I EVER seen or heard of being asked to fill out a form. I think I would have left also.

As for recording, both Chinese and Oriental have security systems that record both video AND audio.

Professional massage folks make you fill out those types of forms, it's nothing unusual.
