Change My Name?

I have not been going by Sarah Storm for at least 3 months. I am not sure how I go about changing it. Sarah was a name I picked quickly for my first job and it stuck, much to my chagrin. It was a tongue in cheek dig at an old rival.
I have been going by Lily Loveless since summer's beginning and I would really like my ECCIE to reflect the change.
