- $500K+ from Medicaid at Minnesota Home Health Care for undocumented/unattended care
- $15M from private insurers via overbilled neurofeedback therapy
- $5.6M from Medicaid for undelivered substance-abuse treatment at Evergreen Recovery
- $211K ($85K paid) from ARMS mental-health program for fake therapy sessions
